Information for Returning Student Athletes

All Returning Bushnell University athletes must complete their online documents in order to participate in practice, games or conditioning drills. TO BE CLEAR: You will NOT be eligible to participate in any activity until these forms are completed.

Please review the information below carefully and complete all requirements as soon as possible, no later than 7/31/26.

Medical Insurance Requirement

All student-athletes must maintain active medical insurance coverage in order to participate in Bushnell University athletics. Student-athletes who cannot provide proof of a valid primary medical insurance plan will not be permitted to participate in practices, conditioning sessions, workouts, competitions, or any other team-related activities until proof of coverage has been submitted. We strongly encourage all student-athletes and their families to carefully review their insurance policy before the school year begins to ensure it provides adequate coverage for athletic-related injuries, including physician visits, diagnostic imaging, emergency care, surgery, rehabilitation, and other sports medicine services that may be needed during participation.

Paperwork Instructions

Your Healthy Roster account from previous years is where you will submit your online documents as well as submit a photocopy of your insurance card. If you cannot access your healthy roster account, try the “forgot password” option.  An email will be sent out on June 12th from Healthy Roster inviting you to complete your forms (please check junk/spam or trash inbox if you cannot find it). The forms can be completed on a computer, but tt is highly recommended that you complete these documents through the Healthy Roster app which you may have downloaded last year,

*As a returner please ignore the “Physical Upload” Document as you are not required to obtain a new physical. Your athletic trainer will upload your previously provided physical on your behalf once you have created your account. 

International Student-Athlete Athletic Training Information

  • International student-athletes are required to maintain a U.S.-based health insurance plan for the entire academic year while enrolled at Bushnell University and participating in athletics. Insurance plans issued outside of the United States often have limited coverage, reimbursement restrictions, or may not be accepted by local healthcare providers. For this reason, international student-athletes must purchase and maintain an American health insurance policy that provides coverage within the United States for the full duration of the school year. (Financial responsibility will fall on the student athlete if not adequately covered for the duration of the school year)
  • VISIT THIS LINK to view and/or purchase international insurance plans from Gallager/United Health Care.
  • For all fall sport athletes, insurance coverage must begin 8/3/26.
  • Failure to maintain an active U.S.-based insurance plan may result in the student-athlete being deemed ineligible to participate in athletic activities until appropriate coverage is obtained and verified.
  • As part of the required paperwork, you must upload clear images of the front and back of your current insurance card through Healthy Roster.
If you have questions about whether your insurance plan meets these requirements, please contact the Athletic Training Staff before arriving on campus so any issues can be addressed prior to participation.
