Information for Returning Student Athletes

All Returning Bushnell University athletes must complete their online documents in order to participate in practice, games or conditioning drills. TO BE CLEAR: You will NOT be eligible to participate in any activity until these forms are completed.

Athletes must maintain medical insurance to participate in athletics at Bushnell University. Athletes who cannot demonstrate proof of a primary medical insurance plan will not be allowed to participate in intercollegiate athletics at Bushnell University. We recommend that you closely examine your current policy to determine if it covers athletic injuries. Please upload a copy of the front and back of your current insurance card (see instructions below).
 
PAPERWORK INSTRUCTIONS: Beginning this year we will utilize a new system called Healthy Roster which is our injury-tracking electronic medical records software. Your Healthy Roster account is where you will submit your online documents as well as submit a photocopy of your insurance card. An email will be sent out on June 16th from Healthy Roster inviting you to create your account and complete your forms (please check junk/spam or trash inbox if you cannot find it). It is recommended that you complete these documents through the Healthy Roster app (Google Play | Apple)  but it can also be completed on a computer as well. *As a returner please ignore the “Physical Upload” Document as you are not required to obtain a new physical. Your athletic trainer will upload your previously provided physical on your behalf once you have created your account.
