Information for New Student Athletes
Welcome to Bushnell Athletics! Before you can participate in any practices, workouts, conditioning sessions, or competitions, all required medical paperwork must be completed and approved by the Athletic Training Staff. TO BE CLEAR: You
will NOT be eligible to participate in any activity until these forms are completed.
Please review the information below carefully and complete all requirements as soon as possible, no later than 7/31/26.
To be eligible for athletic participation, all incoming student-athletes (freshmen and transfers) must complete:
- A pre-participation physical examination by MD, DO, NP or PA (through Healthy Roster)
- All required online medical forms on Healthy Roster
- Insurance verification (through Healthy Roster)
Pre-Participation Physical Examination
Physical examinations must:
- Be completed using the Bushnell University form provided (Bushnell University Pre-Participation Physical Form)
- Be dated after June 1, 2026
- Be signed by both the healthcare provider and the student-athlete where required
- Have all required sections completed. Any missing pages or other forms used will need to be resubmitted correctly.
Please review the form carefully before submission. Missing signatures, incomplete sections, or unchecked required boxes will make the form invalid and may delay your eligibility.
Medical Insurance Requirement
All student-athletes must maintain active medical insurance coverage in order to participate in Bushnell University athletics. Student-athletes who cannot provide proof of a valid primary medical insurance plan will not be permitted to participate in practices, conditioning sessions, workouts, competitions, or any other team-related activities until proof of coverage has been submitted. We strongly encourage all student-athletes and their families to carefully review their insurance policy before the school year begins to ensure it provides adequate coverage for athletic-related injuries, including physician visits, diagnostic imaging, emergency care, surgery, rehabilitation, and other sports medicine services that may be needed during participation.
Healthy Roster Instructions
Bushnell Athletics utilizes
Healthy Roster, an electronic medical records and injury management system. Healthy Roster is where you will:
- Complete all required medical forms
- Upload your pre-participation physical
- Upload a copy of your insurance card
- Manage athletic training documentation throughout the year
An invitation email from Healthy Roster will be sent on
June 12th, 2026 with instructions for creating your account. If you do not see the email, please check your junk or spam folder.
While Healthy Roster can be accessed on a computer, we recommend using the mobile app for the easiest experience.

International Student-Athlete Athletic Training Information
- International student-athletes are required to maintain a U.S.-based health insurance plan for the entire academic year while enrolled at Bushnell University and participating in athletics. Insurance plans issued outside of the United States often have limited coverage, reimbursement restrictions, or may not be accepted by local healthcare providers. For this reason, international student-athletes must purchase and maintain an American health insurance policy that provides coverage within the United States for the full duration of the school year. (Financial responsibility will fall on the student athlete if not adequately covered for the duration of the school year)
- VISIT THIS LINK to view and/or purchase international insurance plans from Gallager/United Health Care.
- For all fall sport athletes, insurance coverage must begin 8/3/26.
- Failure to maintain an active U.S.-based insurance plan may result in the student-athlete being deemed ineligible to participate in athletic activities until appropriate coverage is obtained and verified.
- As part of the required paperwork, you must upload clear images of the front and back of your current insurance card through Healthy Roster.
